HYDE COUNTY HEALTH DEPARTMENT

MEDICATION ASSISTANCE PROGRAM

The Medication Assistance Program is a program designed to help low-income families get their prescription medications at a reduced price or at no cost by meeting the program guidelines, regardless of age.

WHAT ARE THE REQUIREMENTS?

Ø Be a Hyde County or a Northeastern Beaufort County resident

Ø Be on stabilized dosage of medication (Chronic

And Long-term patients)

Ø You do not have insurance coverage for medications

Ø You do not qualify for government or third party programs, such as Medicaid, Medicare Part D, that provides for prescription medications.

Ø You must meet financial requirements of the pharmaceutical companies.

Ø Bring proof of total household income.

Ø Supply a list of current medications.

Ø Must be a US citizen or US resident
